Consortium Services
All DOT regulated employers (class A and class B drivers) must participate in a random drug and alcohol testing program. This includes owner-operators.
We can set you up for this program. It is called a consortium.
Once enrolled, your company will be notified on a monthly basis if an employee or employee’s have been randomly selected to have drug or alcohol testing. They will then come to our facility for testing. The sample will be sent to a DOT certified SAMHSA lab. Results will be sent to you within three days. Your company will then be in compliance with the DOT.
Drug Testing

We perform almost any drug testing you may need:
- DOT Urine, observed if required.
DOT drug testing for random testing programs. - Non-DOT Urine, Lab based or instant tests (results in 5min for instant test).
ETG urine tests for alcohol use within 80 hours.
Pre-employment drug testing.
Court ordered drug testing.
For personal information. - Hair tests for drug use in the last three months.

We are a collection facility for all the labs: Alere, Medtox, LabCorp, CRL, Quest, DrugScan, Omega, Psychemedics, Phamatech, etc.
Paternity Test
Appointment needed.
Providing paternity testing for 10 years.
We use the top lab. All results are 100% accurate.
Tests can be either “Legal” (Court Purposes) or for personal information.
Done with a simple check swab
Legal Tests
100% accurate. DNA lab send-out testing
Lab will provide you with documentation admissible in court (affidavit) stating the results are accurate and whether the man is the father of the child or if he is not.
Complete Fee: $350 (Alleged father & 1 child)
Additional $150.00 for mother or each additional child.
Personal Tests
100% accurate
Our tests are lab based. You will receive documents from DNA lab indicating if the man tested is the father or not.
Results are for your information only and will not be sent to other parties.
Cannot be used in court or to legally establish paternity.
Complete Fee: $240 (alleged father & 1 child)
$130.00 each additional child.
Do not use home tests
Home tests are not always accurate and therefore are not admissible in court. They cannot prove paternity.

Same day appointments usually available. - Dr. McDonald has been certified for 10 years by the US Department of Transportation (DOT) to perform DOT Physicals. Certified Medical Examiner (CME)

- If you use a CPAP bring a 30 day usage printout or app.
- No need to bring forms – We have them all.
